A groundbreaking discovery in cancer treatment has emerged, and it's all about the power of poop! Yes, you read that right. Two studies published in Nature Medicine have revealed the potential of fecal microbiota transplants (FMT) to revolutionize the way we treat multiple cancers.
The first study showed that FMT could eliminate the toxic side effects of kidney cancer drugs, offering a ray of hope for patients. And here's where it gets even more exciting: the second study suggested that FMT could enhance the response to immunotherapy in patients with lung cancer and melanoma.
But how did they do it? Well, the FMT capsules, developed at the Lawson Research Institute, were used in clinical trials at London Health Sciences Centre Research Institute and Centre de recherche du Centre hospitalier de l'Université de Montréal. These capsules, known as LND101, are processed from healthy donor stools and are designed to restore a patient's gut microbiome, ultimately aiding in cancer treatment.
In a Phase I clinical trial, scientists combined FMT with an immunotherapy drug to treat kidney cancer. The results were promising, suggesting that customized FMT could reduce the toxic side effects of immunotherapy. This is a game-changer, as it could allow patients to complete their life-saving treatment without the debilitating side effects.
And the benefits don't stop there. Separate Phase II studies on lung and skin cancer showed that FMT significantly improved the response to immunotherapy. For lung cancer patients, the response rate increased to 80%, compared to the typical 39-45% with immunotherapy alone. Similarly, melanoma patients experienced a 75% response rate with FMT, compared to 50-58% without.
"Our clinical trial demonstrated that fecal microbiota transplantation could enhance the efficacy of immunotherapy," said Dr. Arielle Elkrief, co-principal investigator. "The results also revealed a possible mechanism of action - the elimination of harmful bacteria post-transplant. This opens up a new avenue for personalized microbiome therapies, and fecal transplant is now being tested in the large pan-Canadian Canbiome2 randomized controlled trial."
Dr. Rahima Jamal, Director of the Unit for Innovative Therapies at CRCHUM, added, "Fecal microbiota transplantation in melanoma and lung cancer opens an entirely new therapeutic approach. It's all thanks to the dedication of our patients and the collaborative efforts of our team."
The studies highlight London's global leadership in FMT innovation and treatment. Dr. Michael Silverman, a scientist at Lawson, emphasized, "Using FMT to reduce drug toxicity, improve patients' quality of life, and potentially enhance their clinical response to cancer treatment is a tremendous achievement. And it's all made possible through close collaboration and the exceptional commitment of our patients."
The research builds on previous Phase I studies showing the safety and effectiveness of FMT in treating melanoma. FMT is also being explored for pancreatic and triple-negative breast cancer, and it's already a well-established treatment for serious gut infections like C. difficile.
"Our goal is to help people with cancer live longer, with reduced harmful side effects," said Dr. Ricardo Fernandes, a scientist at LHSCRI. "We're world leaders in FMT research, and we're excited about its potential."
